Bite/Scratch Incident Reports

This document walks you through the features of the Bite/Scratch Incident report. 

Bite/Scratch incidents can be entered either from the Animal Details page (Animal Menu > Add Bite/Scratch Incident) or via a Dispatch (Add Follow Up > from the Follow up page > Add Incident Report (for unknown animal)).

This means a bite report works even without an animal linked to it. 

For that reason, some fields have variable functions, depending on whether an animal is linked or not. 

Mandatory fields will vary depending on your site settings, but the fields marked with an asterisk (*) below are typically required. 

Features of the Bite/Scratch Report

  1. If created via an Animal record, a summary of the animal's information will appear at the top of the page, including Shelter Tag (if present on the Animal Details page), Type, Breed, Age, Gender, Colour, Name, and Animal ID number. 
    1. If created via Dispatch, this information is omitted. 
  2. Type of incident*: This multi-select box lets you select the type of contact involved. To select more than one option, click, press and hold Ctrl, then click again. Options can be added via Admin > Edit Drop Down Lists.
  3. Incident Occurred in: Choose Shelter or Public via radio buttons. These options are not customisable.
  4. Reported By: the source of the report. Options can be added via Admin > Edit Drop Down Lists.
  5. Animal Came In As: identifies the method in which the animal was reported (e.g. seized, impounded, report only, etc.). Options can be added via Admin > Edit Drop Down Lists.
  6. Date of Incident*: The date the incident occurred. This is paired with the Time, found adjacent to this field. There is no default for this field, so users can't overlook setting it, but there is an option to use current time. 
  7. Date Reported*: The date the incident was reported. This may or may not be the same as the above.  
  8. Received by*: Select user name to assign as the one who received the report. 
  9. Quarantine (10 Days) Beginning from: If the animal requires a 10-day quarantine, you can select whether the quarantine period begins on the date the incident occurred or on the date it was reported. These options are not currently customisable. 
  10. Animal Details: If the report is entered via Dispatch, a section appears where you can nominate the Type, Breed, and a brief description of the animal involved.
    If the report is entered via the Animal Details page, this section will not show. 
  11. Source Details: Here, you can nominate who reported the incident. Choose from:

    1. Current person linked to the animal at the time of the bite (only if the report is added via the Animal Details page): This option links to the person who shows via the "Go to Animal Record" at the time the Bite Report is created. 
    2. Staff: If you select this option, a new search field will open, allowing you to search for Shelter Buddy users. Enter at least three characters, and the field will suggest options. Click from the list to select. 
    3. Person ID: Select this option to open a new search field. Enter the Person ID for the Person's record, then click outside the field, and it will show a green success message [Person found: Jane Example]. 
  12. Victim Details:  This section allows you to enter the details of the incident.

    1. Victim Bitten/Scratched: Options available for animals and people. 
      1. As for Source Details, the Staff option will offer a user search, and Person ID (or Animal ID) will find a record using the relevant PID/AID number. 
      2. There are fields available for Person/Animal Other, specify. These fields offer options to enter either a person's name and contact information or an Animal's description. 
    2. Part of Body bitten/scratched: free text field to enter where on the body the bite occurred. 
    3. Bite Scale: A drop-down where you can add standardised bite level identification markers. Options can be added via Admin > Edit Drop Down Lists.
    4. Primary Care Physician Name: If the victim has a regular physician/vet, enter their details here. 
    5. Treating Physician Name: if different from above. 
    6. Clinic: the name of the treating clinic. 
    7. Victim notified of quarantine outcome: a date field to record if/when a victim is notified of the outcome. 
    8. Has received medical attention: simple drop-down for a yes/no answer. 
  13. Parent/Guardian Details: If the victim is a minor or an animal, the contact information for their parent or guardian can be recorded here. There are only two options: None, or Other (with options for contact details). 
  14. Owner Details: The owner of the animal who bit/scratched.
    1. Three Owner options are offered: 
      1. None (the animal is wild, feral, or there is no known owner)
      2. Person ID: search field to link to a Shelter Buddy Person record. 
      3. Other: Opens text fields to record a person's name and contact information. 
    2. Owner Notified of Quarantine Outcome: if the animal is deceased or in care, this is a simple date picker to confirm when they are notified of the result. 
    3. 24 Hour Notice Issued: simple checkbox to confirm this happened. 
    4. Vaccination Dates
      1. If linked to an Animal Details record, these fields will pull from the most recent Rabies vaccination details. 
      2. If not linked to an Animal Details record or if the animal has no vaccinations recorded, these can be entered manually.
    5. Rabies Tag Number
      1. If linked to an Animal Details record, these fields will pull from the Rabies Tag field on the Animal Details page.
        Must be present before the Bite/Scratch incident is created to show. 
      2. If not linked to an Animal Details record, or if the animal has no tag recorded there, these can be entered manually.
    1. License Number
      1. If linked to an Animal Details record, these fields will pull from the most recent Licensing information or from the License Details Found on Animal field. 
        Must be present before the Bite/Scratch incident is created to show.
      2. If not linked to an Animal Details record, or if the animal has no license recorded, these can be entered manually.
  1. Place Incident Occurred: This can be the same as one of the attached people, or somewhere unrelated. Options include:

    1. Dispatch Location: uses the location of the linked Dispatch job (Dispatch job linked only)
    2. Current Person linked to Animal Address: uses the Physical Address of the Person linked to the Animal record at the time the bite record was created. (Animal Details linked only)
    3. Other: If the place the bite/scratch occurred was neither of the above, use this option to record the address in the fields provided. 
  2. Incident Details: A list of standardised questions that offer either check boxes, fields for brief information or comment forms to enter detailed information.
    No customisation is available for this section.
  3. Quarantine Information: A list of standardised fields to record and track quarantine start and end dates, check-ins, release information and observations along the way. There is limited customisable information available in this section.
    Options can be added to the following fields via Admin > Edit Drop Down Lists
    1. Quarantine Type
    2. Quarantine Length (check boxes at the bottom of this section)
  4. Laboratory Details:  A list of standardised questions that offer either check boxes, fields for brief information or comment forms to enter detailed information.
    No customisation is available for this section.
  5. Digital Signatures are available once the form is completed.
  6. When all information has been added, click [Update Details] to save your progress. 

This form can be updated and edited at any time by finding the record the same way it was added (i.e. via Animal Details or by Dispatch Follow Up) > Click the (H) history link > Click Edit. 

All fields with data should appear on the printed page.
